INGESTION BUILDER DECISION — X-Plan-Tier v1: send `free` fail-closed and defer tier plumbing. The DocuClear tier model already exists conceptually, but this first gatekeeper-ingestion integration should not invent deployment config or customer-plan wiring. Free tier's concurrency-1 limit is acceptable for the current phase; enforce service limits and fail closed. Add plan-tier propagation later when billing/plan entitlements and per-customer slug/concurrency accounting are implemented. Keep the service's tier boundary explicit so moving from free to configured tiers is a controlled change.
